It is alleged about a week later Ms T placed "a threatening notice" above Ms Davies's desk.
The documents claim principal Lois O'Reilly gave Ms Davies a "retaliatory" notice and suggested she put that up.
Education Queensland has also acknowledged a cheating allegation involving students being given more time on a test in 2008 was investigated by the Queensland Studies Authority.

The Courier-Mail revealed that Indooroopilly State High School was one of 115 state schools that a recent audit found contained Asbestolux, a potentially deadly low density board ( LDB ).
The science block at Indooroopilly SHS had been completely refurbished.
But "The damage has been done to potentially thousands of students and hundreds of staff, so it's small comfort now to say it's been fixed," Rob Wiltshire said.
Mr Wiltshire had taken photographs of the ISHS science block over the past two years.
His photographs show cracked, broken and water-damaged asbestos ceiling tiles in corridors, classrooms and staffrooms in the ISHS science block.
Mr Wiltshire is in the process of launching civil action against the State Government over alleged physical and psychological damage he says was caused by the school's treatment of him regarding asbestos.
A former staff member is suing this teacher for defamation.
Education Queensland Metropolitan director Chris Rider confirmed that Indooroopilly State High School had contracted an outside consulting company, run by former Education Queensland director-general Frank Peach, to conduct mediation following staff conflict.

14 July 2015 update : I understand that Rob Wiltshire has been found to be "fit for work" by three Qsuper psychiatrists.
Qsuper have also found that Rob is NOT entitled to income protection
- because they advise that he can return to work.
The Department's psychiatrist, however, found that Rob has a personality disorder that cannot be cured by any type of treatment.
- because he won't stop talking about the asbestos problems at Indooroopilly SHS.
So Rob has been forcibly retired.
30 July 2015 update : the Queensland Department of Education were contacted by email on 14 July 2015 and invited to respond to the above allegations.
No response has been received to this date.
